Liz Brooker is a great author on transition
Fraser Brown is fab on Play
School readiness is a crock, other than the current government you'll struggle with this.

School readiness is just a term that will be used in the revised EYFS. it does not mean however that those who look after the under fives start sitting them down forcing literacy and mathematical skills via template and rote counting all the way etc. The emphasis is that children should enter school having a well rounded play based curriculum having had a good well rounded access to PSED, Physical Development and Communication and language and all other areas of learning - with supportive and sensitive adults who understand child development and can provide suitable play actitivities to support each individual child. I am in a rush so probably not explaining this well - but take a look at the attached for your info. Its a start - just google!
