Hi,
we are in the process of changing our medication consent forms after being advised that we have too many! and are been over cautious.
We have a consent form that details name, dosage and signatures for weekly monthly and daily medicine and seperate log that parents sign when they have collected child to say medicine has been administered.
We are looking at having one form per child that all prescribed and non prescribed (eg creams for nappy rash) medicine would be recorded; this will have date, name of child, dosage, etc staff and witness staff will record and signwhen administered and parents upon collection. however wanted to check if ur parents sign consent every am and then at end of every day?
Any response much appreciated!
